Leyden knows that a little help can go a long way. Our Senior Citizen and Persons with Disabilities Programs (funded by Age Options) strive to improve the quality of life for those in our community. We do this by providing services and referrals designed to foster independence, enabling the individual to remain active members of the community for as long as possible.

The following information and services are provided. Seniors 60 and over or persons with disabilities aged 18 to 59 are elegible for services. All services are offered in English, Spanish, and Polish. Office hours are Mon - Fri, 9:00 am - 5:00 pm. Saturday appointments are available upon request. Walk-in's are available, but appointments are preferred.

ADRN serves as a single point of entry to Information and Assistance (I&A) which is giving adequate referrals and information on different benefits or services. Options Counseling (OC), which is to help people plan for their future long term support needs. Senior Health Assistance Program (SHAP) which is providing information and help with applying for various benefits and programs for older adults and persons with disabilities.

Senior Opportunities and Services
This program is designed to identify and meet the needs of older, at-risk individuals through effective referrals to existing health, employment, housing, legal, consumer, transportation and other services. In-house referrals for psychiatric and mental healthcare are available when such needs are identified.

Transportation Services
Transportation services enable individuals to travel to and from community resources in order to receive services, reduce isolation and otherwise encourage independent living. These services provide local transportation for medical appointments and necessary errands, and our drivers provide door-to-door service.
